Abstract

A series of case studies, including the Herald of Free Enterprise sinking, the Longford Gas Plant explosion and an error is medication delivery, are examined to determine why people are not reliable. Physical ergonomics, cognitive ergonomics, organisational ergonomics and environmental ergonomics are considered as ways to increase the reliability of the actions of people at work.
